    SPARTAN ATHLETIC INFORMATION
     
    Eligibility and Clearance  
     
    In an attempt to streamline the athletic clearance process, Sossaman Middle School is going paperless.  Using the enclosed instructions, you will now complete the athletic clearance packet electronically at Register My Athlete.  All forms can be viewed and signed electronically, and additional required forms (i.e.-physical forms and concussion test results) can be uploaded to your account.  Please follow the enclosed instructions to register your students for athletic clearance, for the 19-20 school year.  It is strongly recommended that all documents be submitted electronically prior to the break that precedes your child's sport season (or prior to the school year beginning if participating in a first quarter sport).
     
    Only physicals completed on AIA athletic clearance page will be accepted.  If you have any questions contact Sossaman Middle School. 
     
    The following are acceptable forms for clearance needing completed by a medical professional. All other documentation will not be accepted. 
     
    19-20 Physical FormsPhysical Form A - To be completed and signed by the parent or guardian and student athlete. This form must also be signed by the examining medical provider.  Physical Form B to be completed and signed by the examining medical provider. If completing after 3/1/2019 the 19-20 AIA Physical forms are acceptable. 
      
     
    * ALL STUDENT ATHLETES ARE REQUIRED TO HAVE A COMPLETED ATHLETIC ELIGIBILITY PACKET SUBMITTED TO REGISTER MY ATHLETE PRIOR TO PARTICIPATING IN A SPORT/TRYOUT. *  
     
    *MAKE SURE TO VISIT THE SAFETY AND CONCUSSION TAB IN ORDER TO SUCCESSFULLY COMPLETE THE CONCUSSION PORTION OF THE RMA UPLOAD.* 
     
    *ONCE SCHOOL RESUMES (from summer or break) ONLY FORMS SUBMITTED PRIOR TO THE END OF LUNCH (1:45 PM) WILL BE CLEARED FOR THAT DAY.  ANY FORMS SUBMITTED SUBMITTED LATER IN THE DAY WILL MOST LIKELY NOT BE CLEARED UNTIL THE FOLLOWING BUSINESS DAY.  
     

     
    Season 2 Tryouts 
    Student athletes will not be allowed to tryout without prior clearance in Register My Athelte (see section above).  Students who miss tryout days will not be given an opportunity to make up tryout days.  Please make sure all paperwork is completed prior to the beginning of the school year. 
     
    Boys Basketball - 8th graders: Tuesday, October 15th 7:00am until 8:30am thru Thursday, October 17th in the gymnasium.  
            7th graders:  Tuesday, October 15th 4:00pm until 5:30pm thru Thursday, October 17th in the gymnasium.
     
    Cross Country - This is a non-cut sport. Practice for all those interested and have appropriate clearance will begin on Tuesday, October 15th  4:00pm until 5:30pm on the football fields.  Athletes are highly encouraged to wear running shoes and carry water with them at times. Short informational meeting held Monday, September 23rd in room 239 at 8:00am. Students not in attendance of the meeting are encouraged to see Mrs. Burke prior to the first day of practice to collect the materials. 
     
    Girls Softball -  Tuesday, October 15th 4:00pm until 5:15pm thru Thursday, October 17th on the softball fields.
     
    All tryouts are subject to postponment and/or extension upon coaches' discretion.  
     

    IMPORTANT PARENT INFORMATION 

    Prior to the beginning of the athletic season, it is important to know what you and your athlete should expect as the season begins.  Please take time to review the parent meeting power point (bottom-left tab on SMS athletics homepage) with your athlete.  Once you have reviewed the presentation, please email Brandon.Keeling@husd.org with any questions or clarifications.  Additionally, once your son or daughter has been extended a position on their respective team, the cost for participation is $100 per sport payable to HUSD #60.  This fee is non-refundable.  Please note, uniforms will not be issued until payment has been received in full.


    QUARTER 2 SPORT PICTURES
     
    Our Quarter 2 athletic pictures have been scheduled for Wednesday, November 6th at 4:00pm.  Please help your student athlete to remember to bring their uniform on this day. 

    Remember that uniforms will not be distributed until the athletic fee has been paid.

    QUARTER 1 SPORT BANQUET

    The Quarter 1 Athletic Banquet will be held on Tuesday, September 24th at 4pm in the Student Union.  Should you wish to contribute a snack for the event, we encourage you visit, donate.  

    QUARTER 2 SPORT BANQUET

    Our Quater 2 Athletic Banquet will be held on Tuesday, September 17th at 4pm in the Student Union.  Details to be released as the event draws near.
