    3rd Quarter Athletics

    It is not too early to register for 3rd quarter sports for the 2019-2020 school year!   Sports offered 3rd quarter are boys soccer, girls basketball, wrestling and cheer.  Please add all of the sports for the year that you think your athlete might be interested in.  Be sure to visit the Cooley website under the Athletics tab for all of the information on how to register including the forms needed for a physical and where to go to take the concussion course.  Tryouts begin January 7th and students need to be cleared through Register My Athlete prior to tryouts.  


    Athletic Clearance Process  

    In an attempt to streamline the athletic clearance process, Cooley Middle School has gone paperless.  You will now complete the athletic clearance packet electronically at registermyathlete.com.  

    All forms can be viewed and signed electronically.  All additional required forms must be uploaded to your account (i.e. physical forms and concussion test results). 
    Please follow the instruction link below to register your student for athletic clearance for the 2019-2020 school year.
    Please note that physicals must be dated after March 1, 2019 to be used for the 2019-2020 school year.
    It is strongly recommended that all documents be submitted electronically prior to the break that precedes your student's sport season.  Incomplete registrations will prohibit your student from participating in try-outs. 



     Register My Athlete Instructions

    Concussion Awareness Instructions 


    To complete the AIA Concussion Awareness and Quiz, please go to http://aiaacademy.org.  You will need to create a login and password and register as a 9th grade student at Williams Field High School.  Upon completion of the test, you will receive verification of completion that will included the students name, Arizona Interscholastic Association, and final score in the way of a percentage, and a confirmation code.

    **If your student took the concussion course in 7th grade, please upload a copy of that certificate.  Copies can be found in your account at aiaacademy.org. The course does not need to be repeated. 

    Medical Insurance

    All athletes are required to have medical insurance.  If your athlete is not covered by medical insurance, HUSD has partnered with Myers-Stevens, and Toohey to provide student accident insurance at an affordable price.  Please visit https://www.myers-stevens.com/enrollment-page for more information.
